years as a non-profit home healthcare agency, with our roots as
the original Bridgeport VNA and the original Southbury/Roxbury VNA.

We Offer a Full Scope of Services in Your Community
We have continually expanded the scope of our services to reflect the changing needs of our communities. Today, we offer a wide range of services to address chronic care, acute and post-surgical care, palliative and hospice care, as well as daily health monitoring. We provide services in over 50 communities throughout Fairfield, Litchfield and New Haven Counties.
We Have Areas of Special Clinical Expertise
Talent and training count. Although every one of our nurses, therapists, social workers and aides receives a comprehensive orientation and ongoing training, we also feel that it is important to have specialists on staff. Our specialists act as resources to provide consultation on complex or multi-disciplinary cases. We are proud to have certified wound and ostomy care nurses, infusion therapy specialists, a dedicated telehealth nurse, specially trained hospice nurses and a bereavement coordinator available to augment a patient’s care team.

VNS receives over 10,000 patient referrals each year from facilities
and physicians throughout Connecticut, New York and surrounding
areas. We are proud of the trust that is placed in us to care for

We Are Certified by Medicare, Licensed by the State of Connecticut and Accredited by CHAP
There are many agencies that do not go to the lengths that VNS does
to ensure that it meets the stringent guidelines necessary to be
Licensed and Medicare Certified in the State of Connecticut. We
do. We go further by voluntarily participating with the Community
Health Accreditation Program (CHAP). This accreditation ensures
that VNS maintains the highest standards of home health care practices.
We Have Tough Recruitment Standards
At VNS, we don’t believe in hiring just anybody. For employees who meet our skill requirements, we have a rigorous background check and orientation process to ensure that they are up to the standards of our agency. We also have another set of standards that are a de facto code of conduct for all of our employees — our HEART Values.
We Provide Intensive Staff Training
VNS orientation and training for employees has received state-wide
recognition. Clinical staff who join our team participate in extensive
training with other skilled and experienced staff before they are
given their own cases to manage.
A Comprehensive Handbook is Provided to all Patients
How can we be sure that our patients understand our services, their
rights and responsibilities, our fees, confidentiality requirements,
complaint and emergency procedures? We have created a user-friendly
“Handbook for Patients and Caregivers,” which is provided
to every patient to keep when they come on service with our agency.

We treat home care like any other medical science. Our clinical staff and Professional Advisory Committee maintain strict protocols and clinical guidelines. The staff at VNS understand and follow these guidelines to ensure the delivery of consistent quality care.

We use a highly-respected research firm to measure and
evaluate our success. These measurements are based on how our patients
feel about the care they receive. We use this data for continuous
improvement.
Our Patient Outcomes Exceed National Standards
Medicare reviews the outcomes of patients who receive home care
and how many of them were readmitted to hospitals or nursing homes.
VNS consistently exceeds national standards in patient outcomes
and typically is ranked among the highest agencies in Connecticut.

AT VNS, we carefully assess each individual patient’s needs

exactly the services needed, we’ll help find an agency that

AT VNS we accept patients regardless of their ability to pay. Our financial counselors will thoroughly explore all possible avenues of reimbursement on behalf of our patients, and when necessary, will help with other financial arrangements.
We Are Bridgeport Hospital’s and St. Vincent’s Medical Center’s Preferred Home Care Provider
We are the preferred home care provider for both of these highly
respected acute care facilities. This is a reflection of the value
that is placed upon us as a member of their continuum of care and
it provides VNS with access to a wide circle of clinical expertise
that we may draw upon when necessary.
Private Duty Services
